Responsibilities:

• Oversee and manage patient care plans, ensuring timely and appropriate medical services.
• Act as a liaison between patients, all BCC stake holders
• Organize and schedule patient appointment, follow up any referral related to customer journey
• Ensure adherence to healthcare regulations and standards.
• Maintain accurate and detailed patient records.
• Assess patient needs and resources, identifying and addressing gaps.
• Provide education and resources to patients and their families about care plans.
• Work with multidisciplinary teams to optimize patient care.
• Track patient journey

• Proficiency in English language
• Excellent communication and interpersonal skills.
• Problem-solving and critical-thinking skills.
