Senior Associate - Patient Reported Outcomes Coordination

Job Description

Role Purpose: 

To support value based care operations in operational workflows, analyze patient-reported outcomes (PROMs) and collaborate across teams to optimize patient outcomes, ensure smooth execution of VBHC projects cost effectively.

 

Key Accountabilities:

1-VBC Operations:

  • Assist in developing and executing operational plans for VBHC programs to improve workflow efficiency and care outcomes.
  • Track PROMs implementation and quality metrics to monitor performance trends and support continuous improvement.
  • Support in creating dashboards and reports to facilitate data driven decisions.
  • Identify gaps in VBC operational workflows and suggest enhancements to optimize workflow processes. 

2-Digital Solutions Enhancement:

  • Assist with digital enhancements for VBC data management to streamline information flow and enable real time access to insights.
  • Ensure the accuracy and scalability of systems to support program expansion and reliable analytics.
  • Support innovative solutions to healthcare challenges by piloting new tools and offering feedback for system improvement.

3-Patient Recovery Tracking:

  • Ensure PROMs recovery from patients who miss initial completion by initiating follow-up calls that support participation.
  • Conduct follow up calls/messages using structured scripts to encourage PROMs completion and reinforce the importance of patient input.
  • Address and document patient-reported barriers to PROMs completion to inform targeted strategies for improving response rates.
  • Provide assistance for technical challenges to minimize drop offs due to system issues and enhance patient experience.

  • Ability to navigate through software applications.
  • Ability to multi-task in a fast-paced changing environment and work well under stress
  • Good trouble shooting, problem-solving and analytical skills
  • Excellent follow up skills
  • Good communications skills
  • Good organizational and data analysis skills.
  • Familiarity with data visualization tools (e.g., Excel, Power BI, Tableau).
