To promote and maintain the overall health and wellbeing of individuals, customers and communities with an aim to prevent the onset and progression of diseases through various interventions and strategies
Health Assessment Strategy and Overall Mind set
- Liaises with clinical guidelines and technology to create comprehensive health assessment tools to identify risk factors, evaluate lifestyle to assess overall health status.
- Developing customized and comprehensive preventive health programs tailored to specific population or target group or on individual base focusing on areas like health Screening ,regular check up ,well being of chronic diseases patient and life style modification through nutrition , physical activity and smoking cessation
- Health Education and Promotion
- Identify the potential health risk among the group of customers based on their condition and behavior by analyzing data, trend and surveys and develop strategy to mitigate the risk.
- Conducting Screening for various disease (Cancer-chronic diseases.)
- Conducting regular check up programs based on special criteria and targeted population
- Facilitate immunization programs
- Creating customized healthy Life style program for chronic and non chronic customers based on age /gender and health status
- Collaborating with Health Care providers to coordinate preventive care pathway to ensure seamless Integration of the service
- Monitoring Health out come by tracking KPIs and identify area of improvement and Program redesign
- Policy Development contribute to preventive care policies by providing evidence based recommendations
- Net work collaboration with different organization like stake holders and governmental agencies to foster partnership and support health initiative to promote culture of wellness
Preventive Care Operational Excellence
- Creating comprehensive preventive care programs that out line specific preventive care services and establish protocol and guide line
- Educate and train stake holders on importance of preventive care and their knowledgeable bout specific services
- Implement effective communication strategies to inform the customers about importance of preventive care and encourage them to participate by using different channels as social media.
- Monitor compliance with preventive care guidelines and regularly assess the partners and stake holders.
- Establish analyzing and data collection system to provide patient out come and program effectiveness and make informed decision.
- Collaboration with other stake holders, community partners and insurers
- Identify barriers that prevent the access for preventive care services and develop strategy to overcome it
- Engage in researches activities related to preventive health including Data collection and analysis , technology implementation
Preventive Care Projects
- Support the delivery of preventive care projects by conducting follow up and coaching of our members for all healthcare projects and initiatives launched
Preventive Care Technology
- Ensure availability of technology system as a comprehensive set of tools, devices and software that are designed to promote and support preventive care, system will utilize advanced technologies to monitor, assess and manage individual’s health proactively.
