Job Description
Provides comprehensive geriatric care across both in-person clinic and telemedicine settings, with a primary focus on older adults, addressing the unique medical, functional, cognitive, and psychosocial needs of the geriatric population. Evaluates, diagnoses, and manages a wide range of acute and chronic medical conditions, including patients with multiple comorbidities, polypharmacy, frailty, dementia, and other age-related health concerns. Delivers evidence-based, patient-centered care across both in-person and virtual consultations, ensuring continuity of care, medication optimization, preventive care, and chronic disease management regardless of consultation channel. Coordinates seamlessly between face-to-face and virtual encounters, applying population health and value-based care principles to improve clinical outcomes, optimize healthcare resources, and deliver safe, efficient, and high-quality care for the aging population
Patient Consultations and Management
· Deliver comprehensive care for older adults through both in-person clinic visits and virtual assessment, diagnosis, treatment, and follow-up of acute and chronic medical conditions.
· Conduct comprehensive clinical evaluations, including history taking, physical or virtual assessment, medication review, and geriatric assessment where appropriate.
· Determine, per clinical protocol, when a patient's condition requires an in-person visit versus when it can be safely managed virtually, and facilitate smooth transitions between the two.
· Develop and implement individualized, evidence-based management plans, including medication optimization, preventive care, lifestyle interventions, and referrals when indicated.
· Manage complex geriatric conditions, including multimorbidity, frailty, cognitive impairment, and polypharmacy, ensuring continuity and coordination of care across both channels.
· Collaborate with multidisciplinary healthcare teams, caregivers, and community services to deliver integrated, patient-centered care in-person and virtually.
· Utilize digital health technologies alongside standard in-clinic systems to provide safe, high-quality consultations while maintaining accurate and timely clinical documentation.
· Promote healthy aging through preventive care, chronic disease management, population health initiatives, and value-based care principles to optimize patient outcomes and healthcare resource utilization.
Quality Assurance and Compliance
· Communicate effectively with patients, families, and caregivers, providing clear guidance on diagnoses, treatment plans, and follow-up care in both settings to support informed decision-making.
· Ensure compliance with clinical, ethical, regulatory, and telemedicine standards while maintaining accurate, timely, and complete medical documentation regardless of consultation channel.
· Maintain current knowledge of evidence-based practice, in-clinic and telemedicine regulations, and geriatric care through continuous professional development.
· Contribute to clinical governance by participating in quality improvement initiatives, clinical audits, research activities, and regulatory compliance programs across both service lines to enhance patient safety and care quality.
Professional Development
· Maintain and continuously enhance clinical knowledge in family medicine with a focus on geriatric care, covering both in-person and telemedicine best practices.
· Participate in relevant conferences, workshops, and continuing medical education (CME) activities to remain current with evidence-based practices and emerging advancements in geriatric medicine and telemedicine.
· Integrate the latest clinical evidence and digital health knowledge into practice to continuously improve the quality, safety, and effectiveness of patient care across both channels.
Telehealth Platform Proficiency
· Maintain accurate, complete, and timely clinical documentation in the electronic health record (EHR) in compliance with organizational policies and applicable data privacy regulations, across both physical and electronic systems.
· Demonstrate proficiency in the clinic's EHR system as well as telemedicine platforms and digital health technologies, including video consultations and secure communication, to ensure efficient and high-quality care in both settings.
· Participate in ongoing training and adopt platform enhancements and digital health best practices, collaborating with relevant support teams to optimize system performance and user experience
Patient Safety and Satisfaction
• Uphold the highest standards of patient safety by delivering safe, evidence-based care and adhering to established clinical safety protocols across all patient interactions, in-person and virtual.
• Identify, report, and escalate patient safety risks, incidents, and quality concerns in accordance with organizational policies, regardless of consultation channel.
• Counsel patients and caregivers on diagnoses, treatment plans, medication use, potential adverse effects, and follow-up care to support informed decision-making and medication safety.
• Deliver compassionate, patient-centered consultations through effective communication, comprehensive clinical assessment, empathy, and shared decision-making to enhance the patient experience in both settings.
• Monitor patient feedback and contribute to quality improvement initiatives that strengthen patient safety, clinical outcomes, and service excellence across both channels.
Collaboration with Healthcare Teams
· Collaborate effectively with multidisciplinary teams to deliver coordinated, patient-centred care and optimize clinical outcomes across in-person and virtual pathways.
· Provide clinical expertise and recommendations to support evidence-based decision-making and integrated care planning.
· Facilitate clear and timely communication among healthcare professionals to ensure continuity and quality of care regardless of consultation
Skills
· Medical degree (MBBS, MD or equivalent) from an accredited medical school.
· Residency training in Family Medicine from an accredited program.
· Board certification in Family Medicine.
· Fellowship certification in Geriatric.
· Saudi Commission for Health Specialties SCFHS classification and registration as Family Medicine- Geriatrics Consultant.
· Proven experience as a Family Medicine Geriatrics Consultant or similar role, preferably in a hospital or clinical setting.
· Experience in practicing population health management and value-based care approaches.
· Proficiency in using electronic medical records (EMR) and telemedicine platforms.
· Strong clinical judgment and analytical skills.
· Excellent communication and interpersonal skills, with the ability to build rapport and establish effective relationships with patients and colleagues.
· Ability to work effectively in a fast-paced environment and manage multiple priorities.
