Provides comprehensive medical care to adult patients across both in-person clinic and telemedicine settings, with a primary focus on diabetology within the scope of family medicine. Leads the diagnosis, treatment, and long-term management of diabetes and related metabolic disorders, alongside a broad range of acute and chronic medical conditions, ensuring consistent, evidence-based, patient-centred care regardless of consultation channel. Coordinates seamlessly between face-to-face and virtual encounters through multidisciplinary collaboration, emphasizing prevention, early intervention, chronic disease optimization, and continuity of care. Applies population health and value-based care principles to improve clinical outcomes, enhance patient experience, and optimize healthcare resource utilization across both service modalities.
Patient Consultations and Management
· Deliver comprehensive care for patients through both in-person clinic visits and virtual assessment, diagnosis, treatment, and follow-up of diabetes, related metabolic disorders, and other acute and chronic medical conditions.
· Conduct comprehensive clinical evaluations, including medical history, physical or virtual assessment, medication review, diabetes risk assessment, and interpretation of relevant laboratory and diagnostic findings.
· Determine, per clinical protocol, when a patient's condition requires an in-person visit versus when it can be safely managed virtually, and facilitate smooth transitions between the two.
· Develop and implement individualized, evidence-based management plans, including glycemic optimization, medication management, lifestyle modification, preventive care, and specialist referrals when indicated.
· Manage patients with type 1 diabetes, type 2 diabetes, prediabetes, obesity, and other metabolic conditions, while preventing, identifying, and managing diabetes-related complications and associated comorbidities.
· Collaborate with multidisciplinary healthcare teams, including endocrinologists, dietitians, diabetes educators, pharmacists, and primary care providers, to deliver coordinated, patient-centred care across both channels.
· Utilize digital health technologies, including remote patient monitoring and telemedicine platforms, alongside standard in-clinic systems, to provide safe, high-quality care while maintaining accurate and timely clinical documentation.
· Promote preventive care, chronic disease management, population health, and value-based care initiatives to improve glycemic control, reduce diabetes-related complications, and optimize healthcare resource utilization.
Quality Assurance and Compliance
· Communicate effectively with patients and their families, providing clear and concise explanations of diagnoses, treatment options, and follow-up care in both settings.
· Adhere to all relevant medical regulations, ethical guidelines, and quality assurance standards for both in-clinic and telehealth practice.
· Maintain up-to-date knowledge of clinical and telehealth regulations and best practices; participate in quality improvement initiatives and document patient encounters accurately regardless of channel.
· Participate in quality improvement initiatives, research activities, and clinical audits to enhance patient care and outcomes.
· Regularly review compliance protocols and participate in training sessions; collaborate with compliance officers to ensure both physical and virtual practices meet regulatory standards.

· Continuously enhance professional knowledge and skills in family medicine with a focus on diabetology, covering both in-person and telehealth best practices.

Telehealth Platform Proficiency
· Maintain accurate and complete patient records in accordance with HIPAA-equivalent regulations, across both physical and electronic health record (EHR) systems.
· Maintain proficiency in the clinic's EHR system as well as the designated telehealth platform, including video conferencing, secure messaging, and EHR integration.
· Actively participate in training programs and stay updated on platform enhancements and best practices; collaborate with IT support to troubleshoot technical issues
Patient Safety and Satisfaction
· Maintain compliance with all patient safety standards and ensure patient safety across both in-person and virtual clinical interactions.
· Report any patient safety risks or issues to the appropriate staff in the quality department.
· Practice safe medicine to ensure no patient harm in either setting; explain medications, possible side effects, and proper use.
· Maintain satisfactory patient consultations through communication, taking complete history, providing empathy and understanding of the patient's condition and circumstances, explanation of diagnosis and proposed management plan, and answering patient queries.
· Monitor patient feedback and address concerns promptly; implement safety protocols to minimize risks during consultations in both channels.
· Conduct regular training sessions on patient safety for healthcare staff; utilize patient satisfaction surveys to gather insights for continuous improvement.
Collaboration with Healthcare Teams
· Collaborate with multidisciplinary teams to optimize patient care across in-person and virtual pathways.
· Share insights and recommendations to support patient-centered care approaches.
· Facilitate communication between team members regarding patient status and needs, regardless of consultation channel.
· Mentor junior staff and provide training on clinical protocols for both in-person and telehealth delivery
المهارات
· Medical degree (MBBS, MD or equivalent) from an accredited medical school.
· Residency training in Family Medicine from an accredited program.
· Board certification in Family Medicine
· Fellowship certification in Diabetology.
· Saudi Commission for Health Specialties SCFHS classification and registration as Family Medicine- Diabetology Consultant.
· Proven experience as a Family Medicine Diabetology Consultant or similar role, preferably in a hospital or clinical setting.
· Experience in practicing population health management and value-based care approaches.
· Proficiency in using electronic medical records (EMR) and telemedicine platforms.
· Strong clinical judgment and analytical skills.
· Excellent communication and interpersonal skills, with the ability to build rapport and establish effective relationships with patients and colleagues.
· Ability to work effectively in a fast-paced environment and manage multiple priorities.
· Commitment to ongoing professional development and staying abreast of the latest advancements in family medicine with a focus on diabetology, population health management, and value-based care.
